「あなたが無事に帰ってきて、どんなに安心したことか」 (you came back safe — how relieved I was!) — 「〜ことか」 isn't asking a question, but a strong exclamation: “how ~!,” used to pour out a deep feeling (joy, hardship, worry, gratitude, regret…). 「ことか」 is an exclamation, not a question — no answer needed. Though 「か」 looks like a question, 「〜ことか」 isn't asking — it's an exclamation (how …!), needing and getting no answer. どんなに心配したことか = how worried I was! (pouring out feeling), not asking “how much did you worry?” Don't treat it as a real question to answer. Its 「か」 is an exclamatory ending, with a “beyond words, indescribable” lingering.

na-adjectives・nouns add 「な」 — don't drop it. Verbs, i-adjectives attach the plain form directly (待ったことか, 嬉しかったことか); but na-adjectives add 「な」: 大変なことか (○ how hard!), 幸せことか (○). Without a sense of adjective conjugation, it's easy to drop the 「な」 as 「大変ことか」 (×). Always add 「な」 for na-adjectives.

Often echoed by 「どんなに・どれほど・何と」. 「ことか」 is rarely used alone; it's almost always preceded by an echoing adverb to heighten the exclamation's degree: どんなに〜ことか/どれほど〜ことか (how …!), 何度〜ことか (how many times!), 何と〜ことか (how …!, written). The three differ in use, told apart by feel and pairing:

① 〜ことか = how ~! (a strong, personal exclamation, pouring out deep feeling, often echoed by どんなに/どれほど): どれほど苦労したことか (how much I suffered! — pouring out one's own hardship). The point is “strongly pouring out one's own emotion.

② 〜ことだ = ① you should ~ (advice) ② it really is ~ (a mild sigh about a matter): ① 健康のためには、早く寝ることだ (for your health, you should sleep early — advice); ② 困ったことだ (what a bother — a mild sigh). The advice use ① is commonest, attaching to a verb dictionary form, meaning “(you) should ….”

③ 〜ものだ = a sigh/nature/nostalgia (especially ‘verb た-form + ものだ' for nostalgia): 子供の頃、よくここで遊んだものだ (as a child, I used to play here a lot — nostalgia); 人生とは難しいものだ (life really is hard — a sigh about its nature). 「たものだ」 means “(back then) used to … (nostalgia).”

Trick: strongly pour out your own feeling (how …!) → ことか; give advice (should …) → ことだ; nostalgia (used to …) or nature/sigh → (た)ものだ. E.g., 「どんなに感謝したことか」 = how grateful I was! (exclamation); 「感謝することだ」 = you should be grateful (advice); 「昔はよく感謝されたものだ」 = I used to be thanked a lot (nostalgia). The three — one lyrical exclamation, one advice, one nostalgia/sigh — differ in tone and use.
